So, Fred commits a non-violent, white-collar crime which may leave Ethel financially hurt. His being alcoholic, however, prevents a more serious and undesirable things from happening.

The crime: fraud, scam, tax evasion, insider trading, other money-related crime? Intellectual property involved? Traffic incident?

The bad thing that was avoided, would it have affected Fred? Ethel?

So Ethel has a medical condition that causes rashes on her skin due to allergic reactions. Her garden wall is destroyed by Fred, who crashed into it while driving under influence of alcohol. Correct so far?

hm... my idea could still work... with a tweak! Fred has crashed into Ethel's fence. Police and ambulance are called. When the medics arrive and it is obvious that Fred is not seriously hurt, Ethel grabs the opportunity to ask about her own health (feeling rundown or some such thing). Medical investigations show her to be intolerant to milk and dairy products (allergies can develop in later life so no mystery there) Ethel is glad that Fred is an alcoholic since if he wasn't he wouldn't have crashed into her fence, the medics wouldn't have been there and she would have ate that welsh rarebit she was planning for supper.

Does she leave the milk on the table or work top and something falls into it and thus makes her not drink it? if so, is something related to the crash?

So she is too busy and something else? Yes, well you nedc toknow why she was too busy
is the milk itself affected by something? No to do with the accident? to do with something else? caused by the delay in using it?

is the fact that Fred is an alcoholic in any way related to the milk? ie is this a detail we are working out or the crux of the puzzle? The only relevance of him being an alcoholic is that that was why he crashed

So her fence/wall has just been damaged? destroyed? by a drunk driver... you said no to a questiopn about pets and allergens, can we take it that no pets escaped during the making of this puzzle? so besides the usual businesss in this kinda situation (swapping insurance details , phoning the police etc etc) was she busy with an object? herself only? with Fred? with the car? with the fence? with the garden? with plants? trees?

I wasn't thinking of allergies there, just the gathering back in of the escaped chickens or summat like that.

Is it relevant how hurt he was or just that he was physically hurt by the crash? If so, did he suffer a concussion? broken bones?

Did they need to wait for an ambulance? Did he have to be checked out at the scene of the accident (which took a lot of time)? Did he have to go to the hospital?

Did her dealing with the accident (once she heard the crash) take less than an hour? 1-2 hours, 3-4 hours? more?

I am assuming that she is busy with these distractions, her rash disappears (and maybe she doesn't even think about it, what with everything going on), she goes inside and sees a glass of milk (which is probably very warm and probably spoiled), and she thinks, "I forgot to drink my milk," and then she realizes she doesn't have a rash which she has dealt with for a long time, and puts two and two together...."

Ethel was minding her own business one morning when Fred, who'd been out on the town last night, drove his car into her wall. She rushed out, surveyed the damage, tended to Fred's wounds, waited for the ambulance...by which time she was very late for work. She rushed out, no time for her daily bowl of cornflakes (with milk), stopping only to grab a quick snack from the fridge. The next day she notices that that pesky skin condition she's had for a while is starting to clear up, puts two and two together and...
