Is fuel stored inappropriately? Where it's not supposed to be? Are fumes allowed to leak where they are not supposed to go? Is there something wrong with the ventilation system? Did the crew know about the problem? Did they ignore it? |

The problem was with the ventilation, correct? Leading fumes to build up in an area? Then they tried to fix the problem? But it only lead to fumes spreading to other areas? Which then exploded when they were hit? Was something mounted/run backwards? |

Did someone on the ship actively ignite the fumes? If so: by accident? Negligence? did the fumes ignite because: they reached an existing source of ignition? A source of ignition was created in an area where the fumes already were? were the fumes from: ship fuel? Airplane fuel? Other? Relevant? Did something happen to the fuel to make it release more fumes than usual? Such as heating? |

Yes. ******************* Sinking Spoiler: ******************* The Japanese aircraft carrier HIJMS Taiho was attacked in 1944 by the submarine USS Albacore. Albacore fired six torpedoes and only scored one hit, which caused a leak in a fuel bunker and not much else. Taiho's hangars were completely enclosed, with almost no vents, and there wasn't an easy way to remove the spreading fumes (other than the foaming fire suppression system, which no one thought to use.) In desperation, the damage control officer turned the ventilation system of the entire ship on to try to blow the fumes out of the ship through the very few portholes. Instead, they spread throughout the ship and turned her into a floating time bomb. Six hours after taking minor damage that should have been easy to repair, Taiho exploded and sank with most of her crew. |
