Teachers always tell students to check their sources and proofread their work. Some people recently failed to follow this rule, and thus caused a lot of embarrassment. How?

Note: This is based on a recent news story, so no GooglePedia on this one please.

Are the people students? Did they both fail to check their sources and fail to proofread their work? Did all the people produce the same work? i.e. were they working together to produce something? Or are we dealing with a series of individual pieces of work?

Was the embarrassment caused to the "people"? To anyone else?

Did any of the sources not actually exist? Were any of the sources fictional characters? Did the people include something in their work which they would have removed if they had proofread it? Did they fail to include something they would have included if they had proofread it?

Yes i.e. were they working together to produce something? Or are we dealing with a series of individual pieces of work?

I should add that their failure to follow the rule is applied to a real-life, non-academic situation. So don't get caught up in that.

Did their "work" contain words? Numbers? Images? Was it viewed by members of the public? Was it presented online? On a social network? In the news? In print? Was it presented orally? In song?

Are the people politicians? A music group?

Are the other people who were embarrassed quoted in the work? Mentioned in the work? Are the other people who were embarrassed in any way related to the people who did the work? Are there a lot of other people who were embarrassed? A small number? A whole country?

And I really ought to ask: did the people who didn't proofread/check their sources use Wikipedia or another reference source?

No Are there a lot of other people who were embarrassed? One directly, more indirectly... A small number? A whole country? Indirectly!!

Were they looking up the lyrics of a song? yet they found an incorrect version of the lyrics (e.g. a mondegreen) and sang that in front of a large group of people?

I was hoping this would be an Enrico Pallazzo situation My school library doesn't have "The Naked Gun"; I was hoping to rent it. Ugh.

"Borat's Fake Kazakhstan National Anthem Plays By Mistake During Medal Ceremony"

The song, which was featured in the 2006 Sacha Baron Cohen film, was downloaded in error by event organizers.

During a recent sports event in Kuwait, the wrong national anthem for Kazakhstan played during the medal ceremony.

Instead, it was the spoof from the 2006 satirical film starring Sacha Baron Cohen, Borat: Cultural Learnings of America for Make Benefit Glorious Nation of Kazakhstan, that was heard throughout the arena at the 10th Arab Shooting Championship.

Lyrics from the fake anthem include: "Kazakhstan's prostitutes are the cleanest in the region, except, of course, for Turkmenistan's."

Incredible! Poor girl, working probably hard for many hours/days/months, maybe years, to find out that a bunch of morons managed to do this.

In her place I'd have definitely stepped out and leave in protest.
