At a particular aviation company, there was once a rash of accidents due to workers leaving leftover tools inside the inlets of enormous jet engines. When the engines were turned on for testing, the debris whirled around at high speed and destroyed the valuable machinery. Eventually, it was decided that one engineer would enter the inlet with a flashlight and confirm personally, in writing, that no tools had been left behind. After the next disaster, it turned out the inspector had forgotten his flashlight.

And did this really happen? Was there a news story of some sort that this was based on, or did you make it all up?

It's from a factual, humorous book on engineering goofups, so I assume it occurred at Lockheed or somewhere.
