Damn, time to take another stroll down "limited knowledge avenue". I'd say endochrine but i'm not sure, pretty sure it's not the rest though. In fact to save some unnecessary guessing i'll tell you that it's the liver

I should say that by the end Dennis realises Melvin means the type of liver you would normally eat

The key part of the conversation apart from the order -- it occurred prior to the order itself? Was it a setup for the bogus order? For example, asking Dennis if they make all kinds of pizza?

This key part of the conversation was not something that caused Dennis to suspect that Melvin was calling for any reason other than to order a pizza? Would something like it be a part of any typical phone call to order a pizza?

Was Melvin providing information about himself? His name (presumably a fake)? His address? Was he asking for some information about the restaurant? Its menu? Its prices?

Is some sort of special promotion involved?

Is it relevant that Dennis both answers the phone and makes the pizzas? Was Dennis working alone on this occasion? Would the puzzle work if Melvin were talking to someone who just took orders, but didn't make pizzas? Or did someone else answer the phone, and Melvin asked to speak with Dennis?

Melvin is a radio DJ who does a daily crank call on his show. He picks on Dennis constantly because Dennis is a notoriosly cranky Pizzeria owner. On previous occasions Melvin had rung Dennis and asked if they did takeaways and when Dennis said yes Melvin asked him what 12 minus 7 was.

On this occasion Melvin rings and asks Dennis "Do you deliver" . When Dennis says yes Melvin proceeds to order a liver & onion pizza. Dennis angrily states that they don't put liver on pizzas so Melvin says "But you just said you do liver". Cue Dennis who realises it's another crank call letting go with a torrent of abuse.
