The man holds down his index finger and ring finger. He shakes his head.

The man holds down his ring finger alone. He smiles and nods.

Are the fingers held down on a musical instrument? A piece of mechanical equipment?

Relevant exactly which fingers he uses? Relevent that he nodded for the ring finger in particular? Length of fingers relevant?
Held down= layed on a surface? Lowered more than the other fingers? Touched to the palm?

The man is, indeed, a baseball catcher. The catcher is giving the pitcher signals for different pitches untill he (the pitcher) gets one he likes, when he smiles and nods.
