Were the disobeyed instructions given by the (this) doctor?
Did the disobedience result in any relevant outcome? Good? Bad?
Were the instructions disobeyed immediately after given, or was it some time later?

Were the consequences of the disobedience evident at a first glance? Did the doctor laugh for those? Did he laugh for the reason why the patien disobeyed?
Was the patient ill? Were the instructions related to something he/she needed to do in order to recover him/herself? Did the patient suffer from some disease? Are any of these relevant: medicine? Pills? Hygene? A therapy?

Did the patient meet the doctor in quality of person who needed to be cured/recovered?

Was the patient also someoene who worked with the doctor? A friend of him? Was the patient HAF? HAM? Did the doctor laugh because the disobedience caused something funny? Was caused by something funny? Did patient and doctor meet at an hospital? At a place where usually doctors are found? Did they speak by phone? Sms? Did they know each other outside of the work of the doctor? Doctor = HAM?

Instructions = instruction about "where to go"? "What to do in order to ricover"? "Where to meet"? Instructions told by the doctor? Written on a sign? On a sign written inside an hospital? Outside of a buolding?

"Keep upright" written on the bottom of something, so that when you try to read it, you turn it over? Good idea, but No

Does it specify somewhere you should put it or not put it?

From a chemist, hardware store, other? You can buy this item at a chemist shop/pharmacy (at least, you can in pharmacies in the US; I'm assuming UK chemists have similar stock) Expensive or inexpensive? Inexpensive

When you and some of the other staff are having a slow night and decide to watch GI fish a shampoo bottle out of some guy's "wazoo" with a scope, try not to be rude by laughing hysterically when the words "For External Use Only" are largely displayed on the monitor.
