Does his death have to do with fishing equipment? His rod? Hooks? Bait? Nets? Boat (though he doesn't die on it)? Fish?
Does he catch fish? Something else, like crabs? Relevant?
Is he sleep deprived?

Lifted by crane? None of these. Did something go wrong? Yes. Did he just have more work than he could take? OTRT.

Lifting relevant? Carrying? Putting down? Does he injure his: head? Back? Arms? Legs? Does he fall? Does he think a container is full when it's empty, using far too much force? I heard of someone who permanently injured his back that way.

Lifting relevant? Carrying? This. Putting down? Does he injure his: head? This. Back? Arms? Legs? Does he fall? No. Does he think a container is full when it's empty, using far too much force? No. I heard of someone who permanently injured his back that way. My Dad taught me to be careful. Hence the puzzle title.

Could he have lived if he had worn a helmet? Interesting! Only a few types of helmet; most wouldn't have helped.

WAG: is he carrying buckets from freeze storage aboard the boat, not having enough hands he carries one in his mouth, freezes his tongue to the handle but doesn't notice, drops it at the destination and tears his tongue out, and dies from blood loss?

WAG: is he carrying buckets from freeze storage aboard the boat, not having enough hands he carries one in his mouth, freezes his tongue to the handle but doesn't notice, drops it at the destination and tears his tongue out, and dies from blood loss?

I rather like this solution, but the actual one, while broadly similar, fits the question a little better. This one would work just as well for a freezer technician.

But he is indeed carrying something in his mouth due to lack of available hands.

The fisherman has too many fish to carry, so instead of leaving some and going back for the rest he holds one in his mouth. Unfortunately, it is a piranha, and it is still alive. It bites off his tongue and he bleeds to death.
